The cover shall be securely held in place to prevent tools or materials from falling through.    Drilling Site:  On or about April 11, 2012, an uncovered hole was found on Rig #26 on the elevated walking platform at Mud Pit #2 furthest from the drill.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.    Drilling Site: On or about April 11, 2012, a shower unit was discovered to not have water connected to the unit.  The unit was located on the elevated work platform at the mud pit furthest from the drill.  The hazard exposes employees to corrosive materials without the ability to quick drench or flush themselves off during emergency use.
