The panel door sequential list of circuits up to 42 did not match circuit breaker identification on the panel. b) At the facility in the drug vault exhaust fan mechanical room: On April 10, 2012 the employer failed to ensure the knife switch disconnect for the drug vault exhaust fan was labeled or legibly marked to indicate its purpose.
