29 CFR 1926.760(a)(1): The employer did not ensure that employees engaged in steel erection activity that were on walking/working surfaces with an unprotected side or edge more than 15 feet above a lower level, were in use of fall protection: Employees were observed on the west side of the roof of a building without fall protection. The employees were exposed to a fall of greater than 15 feet to the ground below.

29 CFR 1926.1053(b)(1): On or about April 10, 2012, the employer did not ensure that portable ladders used for access to an upper landing surface, extend at least 3 feet above the upper landing surface to which the ladders are used to gain access: Employees were observed using portable ladders to access an upper landing surface, when the ladders did not extend at least 3 feet above the upper landing surface. This exposed employees to a fall hazard of approximately 14 feet to the ground below.
