29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Fisher & Ludlow, Inc. / 607 Erie Street, Saegertown, PA--- Littell Machine #412-7HD Serial 86885*82*1: Guarding devices were not arranged in a manner to keep the hands out of the machines moving parts and pinch points while feeding coil into the straightener.  The moving parts/pinch points included but were not limited to the coil/positioning rollers.  In addition, the pendant and the control panel jog button was not configured and installed in a manner that would keep hands and other body parts out of the machines moving parts and associated pinch points.
