29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: a) Flight of stairs to infeed operators platform for the Laminate Line located in Plant 1: On or about April 11, 2012 and at times prior thereto, the employer did not ensure standard railings were installed on each open side which exposed employees to falls up to 3-feet. b) Flight of stairs to outfeed operators platform for the Laminate Line located in Plant 1: On or about April 11, 2012 and at times prior thereto, the employer did not ensure the railing system on each open side included a midrail which exposed employees to falls up to 4-feet. c) Flight of stairs to on West side of outfeed belt conveyor for UV FinishLine (5-ft): On or about April 11, 2012 and at times prior thereto, the employer did not ensure standard railings were installed on each open side which exposed employees to falls up to 3-feet. d) Flight of stairs to on East side of outfeed belt conveyor for UV FinishLine (5-ft): On or about April 11, 2012 and at times prior thereto, the employer did not ensure standard railings were installed on one of the two open sides which exposed employees to falls up to 3-feet.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Reverse Roll Coater of Paint Line located in Plant 1: On or about April 11, 2012, the employer did not ensure each employee required to work in the hazardous area during the repair activity affixed a lockout device to the electrical control device which exposed employees to amputation and other hazards associated with the inadvertent startup of the equipment.
