29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Batch Plant; On or about April 11, 2012 and at times prior thereto the employer exposed his employees to potential hazardous energies in that employees were allowed to perform maintenance and servicing on the batch plant without first locking out the system using the procedures established for the plant to prevent accidental startup.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Truck Washing Area; On or about April 11, 2012 and at times prior thereto the employer exposed employees to hazardous chemicals in that employees were allowed to use Citriclean a corrosive cleaning product for removing cement deposits from trucks without facilities for quick drenching or flushing of the eyes and body.
