29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk shall be guarded by either: a) On or about April 3, 2012 in the Hot Food Table department the steel grading in the floor had an opening that measured nine and a quarter inches, an opening in which an employees could get their feet caught with a potential of causing a fall or broken ankle.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. a) On or about April 3, 2012, three spools of electrical wiring and an air tank was located in front of a 208 volt electrical panel box preventing safe working space and access to panel.
