29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operationspresent potential eye or face injury from physical, chemical, or radiation agents. On April 10th, at 1802 79th Ct in Kansas City, MO, employees were exposed to an eye and face hazard while installing roofing materials in that they were operating pneumatic nail guns without the use of eye protection, there was the potential for eye and face injuries due to use of machine and the operations performed.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) ormore above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrestsystems. On April 10th, at the jobsite located at 1802 79th Ct in Kansas City, MO employees were exposed to fall from height hazards in that they were performing re-roofing activities on roof structure approximately 23 feet above ground level without the use of an approved fall protection system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On April 10th at the jobsite located at 1802 79th Ct in Kansas City, MO employees were exposed to fall from height hazards in that they were accessing and egressing the roof structure without the proper use of their ladder. In that the ladder side rails did not extend at least 3 feet above the eve of the roof, nor was the ladder secured at the top.
