(b)   On or about April 12, 2012 at 135 Cloverhill Dr., Montgomery:  The employer failed to ensure employees installing sheathing on a 6:12 pitch roof with a 18 feet 8 inch fall eave to ground were protected from fall which exposed them to the hazards associated with falls.
