29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: (a) Stacker - On or about April 10, 2012, employee was exposed to continuous noise at 102% of the permissible daily noise (8-hour time-weighted average sound level of 90dBA) or an equivalent sound level of approximately 96.9 dBA during the 451 minutes sampling period on 04/10/12. Exposure calculation included a zero increment for the time not sampled. (b) Stacker - On or about April 10, 2012 employee was exposed to continuous noise at 66.2% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 87.0 dBA during the 453 minutes sampling period on 04/10/12. Exposure calculation included a zero increment for the time not sampled.
