29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: On or about April 15, 2012, the wood handle of a sledge hammer was not secured to the hammer head. The hammer was used to make connections and was located at the wellhead. This condition created a struck-by hazard to workers.
