29 CFR 1926.353(b)(3): "Welding, cutting, and heating in confined spaces" Where a welder must enter a confined space through a manhole or other small opening, means shall be provided for quickly removing him in case of emergency. When safety belts and lifelines are used for this purpose they shall be so attached to the welder's body that his body cannot be jammed in a small exit opening. An attendant with a pre-planned rescue procedure shall be stationed outside to observe the welder at all times and be capable of putting rescue operations into effect. Turbine #10 - An attendant and rescue procedure/system were not utilized during cutting operations performed in the confined space.

29 CFR 1926.404(b)(1)(i): The employer shall use either ground fault circuit interrupters (GFCI) as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. In the following instances electrical equipment was not protected with GFCI: a) Generation Plant - The two flexible cords supplying power to the temporary lights. b) Turbine Roof - The Romex cable supplying power to the core drill.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. Turbine Roof - The employer did not develop a fall hazard training certification record.
