29 CFR 1910.145(f)(3): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Truck Staging Area : On or about April 10, 2012 and times prior thereto, a 10 foot ladder was available and set-up for use that had a cracked side rail that was wrapped with duct tape. This exposed employees to a fall of up to 7 feet if the ladder were to collapse during use.
