29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) In front yard of home at 1920 Kenmore Avenue, Bethlehem PA - On or about April 17, 2012 employees were working in an unprotected trench, at a depth exceeding six foot three inch trench with cave in hazards and no protective system in place that had not received a daily inspection to protect workers from trench collapse or other related excavation hazards.
