29 CFR 1910.132(e): The employer did not ensure that defective or damaged personal protective equipment was not used: At Rig 1206 inspection site at Jackson No. 1-18H well in Dewey County, Oklahoma, a flame-resistant Indura jacket provided for employee use while performing well servicing operations had several tears on the torso and sleeves, exposing the lining of the jacket, and potentially exposing the employee to the hazards of thermal burns.

29 CFR 1910.157(c)(4): Portable fire extinguishers were not maintained in a fully charged and operable condition: At the Rig 1206 inspection site at Jackson No. 1-18H well in Dewey County, Oklahoma, where employees were expected to use fire extinguishers on incipient-stage fires, an ABC dry chemical portable fire extinguisher had been discharged and was missing the inspection tag. Basic Energy Services, LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(4) which was contained in OSHA inspection number 315557322, citation number 01, item number 01 and was affirmed as a final order on 02/02/2012 with a final abatement date of 02/01/2012 with respect to a workplace located at O'Brien, G.W. Etal, Well #1886 eight miles west of Monahans, Texas.
