29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from anaerial lift: (a) Southside of Field House - On or about April 2, 2012 an employee was exposed to a 40 foot fall hazard while trimming trees in a FL 80, Freight Liner bucket truck. Employee was not wearing a body harness and lanyard attached to boom or basket.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects: (a) Southside of Field House - On or about April 2, 2012 employees on the ground were exposed to being struck by falling tree limbs and tree branches during tree trimming. Employees were not wearing hard hats.
