29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    a.)  Furmanite America Inc., Shop Area, Electrical Panel #4.  On or about April 12, 2012, the blank cover over breaker #9 did not fully enclose the opening, leaving a gap and exposing employees to inadvertent contact with the energized busbar in the rear of the panel box.

29 CFR 1910.305(b)(1)(ii):     Unused openings in cabinets, boxes, and fittings shall be effectively closed.    a.)  Furmanite America Inc., Shop Area, metal junction box mounted on the wall near Baldor pedestal grinder.  On or about April 12, 2012, the junction box housing the service wiring leading to the grinder, had an unused opening in that the knockout plug was missing exposing employees to inadvertent contact with live parts.     b).  Furmanite America Inc., Valve Storage Room, Lapmaster 12 - lubricating machine.  On or about April 12, 2012, employees were exposed to live parts in that openings were not fully enclosed where the flexible cord entered the machine exposing the connection to the operating switch.

Furmanite America Inc., Boiler Room, Valve flushing area, Bradley shower/eye wash unit.  On or about April 12, 2012, the employer did not ensure the shower/eye wash unit available for use by employees was in good working order, in that the unit had not been maintained or flushed as evidenced by the dust/debris covering the nozzles and the bowl.

On or about April 12, 2012 the working space in front of the facility's main electrical panels, circuit breaker panels, and equipment disconnect switches was used for storage in that miscellaneous items such as, but not limited to pallets loaded with various-sized valves,  a welding machine, and large boxes were located within 17 inches of the working space.
