On or about April 12, 2012, the blank cover over breaker #9 did not fully enclose the opening, leaving a gap and exposing employees to inadvertent contact with the energized busbar in the rear of the panel box.

Furmanite America Inc., Shop Area, metal junction box mounted on the wall near Baldor pedestal grinder. On or about April 12, 2012, the junction box housing the service wiring leading to the grinder, had an unused opening in that the knockout plug was missing exposing employees to inadvertent contact with live parts. b). Furmanite America Inc., Valve Storage Room, Lapmaster 12 - lubricating machine. On or about April 12, 2012, employees were exposed to live parts in that openings were not fully enclosed where the flexible cord entered the machine exposing the connection to the operating switch.
