29 CFR 1910.23(a)(2):     Every ladderway floor opening or platform shall be guarded by a standard railing with standardtoeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offsetthat a person cannot walk directly into the opening.    (a) Employees were potentially exposed to serious injury from a ladderway that did not have a swinging gate, nor was offset. The ladderway was located in the SE corner of the adult fish facility at Bonneville Dam.

29 CFR 1910.176(b):     Storage of material created a hazard:    (a) Employees were potentially exposed to serious injury from the Rotatin rack system, which was not stable. The Rotatin racks were located in the north section of the warehouse annex at Bonneville Dam.    (b) Employees were potentially exposed to serious injury from the steel racking system which was not bolted to the floor or otherwise secured. This rack system, composed of horizontal and vertical members was locted in the south section of the warehouse annex.    (c) Employees were potentially exposed to serious injury from the use of a rack that had a damaged vertical member. The damaged rack was located between the horizontal sections marked 11-20-18 and 11-20-20 located in the south section of the warehouse annex.     The employer was cited for a violation of the same standard and/or same hazard on 01/26/2011, Inspection #309092500, date 02/28/2011 of Final Order/settlement, and means of abatement.      The employer was cited for a violation of the same standard and/or same hazard on 10/12/2007, Inspection #307507202, date 10/22/2007 of Final Order/settlement, and means of abatement. (b) Employees were potentially exposed to serious injury from the hazard of contact with moving parts due to inadequate guarding on the unused portion of the saw blade on the Saber horizontal metal bandsaw located in the weldshop.    (c) Employees were potentially exposed to serious injury from the hazard of contact with moving parts due to inadequate guarding on unused portion of the saw blade on the Delta vertical bandsaw. The bandsaw, serial #06D13338, was located in the powerhouse 2 mechanic shop.     The employer was cited for a violation of the same standard and/or same hazard on 05/26/2010, Inspection #309097236, date 06/30/2012 of Final Order/settlement, and means of abatement.      The employer was cited for a violation of the same standard and/or same hazard on 11/23/2009, Inspection #312391535, date 12/21/2009 of Final Order/settlement, and means of abatement. 29 CFR 1910.178(a)(5):     The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    (a) The inspection revealed that the Toyota Forklift, model 7FGU25, serial #8321 was not marked to identify the 72" forklift attachments that were available for use. The Toyota Forklift was location in the warehouse at Bonneville Dam.    (b) The inspection revealed that the Komatsu Forklift located behind the weld shop was not marked to identify the forklift attachments that were on the lift.     The employer was cited for a violation of the same standard and/or same hazard on 08/19/2011, Inspection #96892, date 09/15/2011 of Final Order/Settlement, and means of abatement.
