29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about April 10, 2012, the drilling rig located at Lavvorn Buford Lease, Well #1, Bartelo Moreno Well Service Rig #1, 6 miles N. of Stamford, had no railing at the operators position where the employee was standing. This condition exposed the employee to a fall hazard of approximately 52 inches to the ground.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: On or about April 10, 2012, the employees at Lavvorn Buford Lease, Well #1, Bartel Moreno Well Service Rig #1, 6 miles N. of Stamford, were using an unsecured ladder to access and egress from the operators position.This condition exposed the employees to a fall hazard of approximately 52 inches to the ground.
