29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. (a) Jobsite- 410 Llyod Avenue, Latrobe, PA: On or about April 6, 2012- employees were installing shingles approximately 20 feet above the ground on a 6:12 pitched roof and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards. (a) Jobsite- 410 Llyod Avenue, Latrobe, PA: On or about April 6, 2012- employees that were working on a residential roof that measured 20 feet from the ground to eave, having a 6/12 pitch and were not provided with fall hazard training.
