29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Foundry - On or about April 10, 2012, foundry employees were exposed to a corrosive liquid, (United Erie Catalyst NPT-S-NM) containing Toluenesulfonic acid, Sulfuric Acid and Fluoboric Acid without a nearby eyewash or facilities for quick drenching of the body.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet: a) Maintenance Shop - On or about April 17, 2012, four oxygen cylinders were located approximately 12.5' away from a diesel fuel tank.
