29 CFR 1910.94(a)(4)(i): The construction, installation, inspection, or maintenance of exhaust system(s) did not conform to the principles or requirements set forth in American National Standards Institute Fundamentals Governing the Design and Operation of Local Exhaust Systems Z9.2 1960 and ANSI Z33.1 1961: a) On or about April 18, 2012, at the above located workplace employees were required to work with and around a natural gas fired power washer that released exhaust gases into the building. The exhaust gas ventilation system did not meet the principles or requirements set forth in the American National Standards Institute Fundamentals Governing the Design and Operation of Local Exhaust Systems Z9.2 1960 and ANSI Z33.1 1961. Employees were thereby exposed to the hazards associated with exhaust gases including but not limited to carbon monoxide.

The power washer was missing a cover and had exposed energized (120 volt) wiring. Employees were thereby exposed to hazards associated with 120 volts of electricity. b) On or about April 18, 2012, at the above located workplace, employees in the in the shop were required to work with and around a on an Emglo air compressor. The air compressors electrical pressure cutoff switch was missing a cover and had exposed energized (240 volt) wiring. Employees were thereby exposed to hazards associated with 240 volts of electricity.
