29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: (a) Employee(s) operating the Marvel 8 bandsaw in bay 2 were exposed to physical injuries due to no guarding being provided for the rotating blade moving inside the open area of the bandsaw frame. The rotating blade has a maximum speed of 1800 rpms. (b) Employee(s) operating the Marvel 8 bandsaw in bay 2 were exposed to physical injuries due to the bottom drive wheel not being fully enclosed on the operator side of the bandsaw. (c) Employee(s) operating the Clausing drill press in bay 2 near column 19 were exposed to physical injuries due to no guard being provided for rotating spindle that spins between 150 - 2000 rpms.

29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): Employee(s) operating the Johnson shear on the full revolution mechanical power press in bay 3 near column 8 were exposed to physical injuries due to inadequate guarding for the point of operation. The opening on the right side guard was approximately 26 inches wide and 3-1/4 inches in height for cutting materials that was approximately 1/4 of an inch, thus allowing entry of hands into the point of operation. SOUTHWEST STEEL FABRICATORS, INC., WAS PREVIOUSLY CITED FOR VIOLATIONS OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR IT EQUIVALENT STANDARD 1910.217(c)(2)(i)(b), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310779756, CITATION 1, ITEM NUMBER 7, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 26, 2007 WITH RESPECT TO A WORKPLACE LOCATED AT 2520 SCHEIDT LANE, BONNER SPRINGS, KANSAS.
