29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths ofwork and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. On or about 12 April 2012 and times prior thereto, Pump Room welding cables had cuts in the insulation improperly repaired with electrical tape. This condition exposed employees to electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 12 April 2012 and times prior thereto, an electrical cord to the oil rig drive controller in the control room had insulation pulled away from the plug end. This exposed employees to electrical shock hazards.
