29 CFR 1926.300(b)(5): "Exposure of blades." When the periphery of the blades of a fan is less than 7 feet (2.128 m) abovethe floor or working level, the blades shall be guarded. The guard shall have openings no larger than 1/2 inch (1.27 cm). (a) At the Northeast corner of apartment foundation slab - on or about April 12, 2012, employees were exposed to injury due to the absence of a guard over the fan blades of the 1.5 horsepower industrial motor powering the Emglo Model KU, Air Compressor, S/N 022588084; the 4.75 inch diameter fan, and the top of the motor were about 16 inches above the concrete slab.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. (a)At the Northeast corner of the apartment building's lot - On or about April 12, 2012, employees walking around extension cords and electric hand tools were exposed to electric shock due to the absence of ground-fault current interrupting protection on the two 20-amp single-pole circuit-breakers in the Midwest disconnect panel. Neither were Ground-Fault Receptacles, nor an Assured Equipment Grounding Conductor Program, used.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not of the three-wire type: (a) Near the center of the apartment building's first floor concrete slab - On or about April 12, 2012, employees were exposed to injury due to the use of a yellow 2 foot-long, AWG 12/3, 3-way extension cord which was missing the grounding prong at its male attachment plug. It was supplying power to a double insulated Rigid 7 1/4 inch Skilsaw.
