29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    (a) On or about April 10, 2012 rotating shafts in the Wrap Room on Line 1, Line 2, and Line 4 were not guarded.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidentalcontact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    (a) On or about April 10, 2012 In the Refrigrator Room a 480 volt panel box with open spaces was not effectively closed exposing employees to electrical shock.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    (a) On or about April 10, 2012 the electrical wiring on the Thumbler was not continuous resulting in the strain release being broken exposing live wiring.
