29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a) Jobsite:  On or about April 4, 2012, an employee was working (installing asphalt shingles) from a residential roof and he was not protected from falling from the roof.   The fall hazard ranged from approximately 12 feet to 20 feet.

29 CFR 1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a) Jobsite:  On or about April 4, 2012, an employee who was  positioned on the ground working beneath roofing operations overhead, was not wearing a protective helmet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Jobsite:  On or about April 4, 2012, the employer did not provide fall protection training to each employee exposed to fall hazards while working on a residential roof.  The fall hazard to the employees ranged from approximately 12 to 20 feet.
