(a) Dan Thornton Construction on or about April 17, 2012, at the address of 5301 W. Alexis Rd. Sylvania, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting siding work above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. a) Dan Thornton Construction on or about April 17, 2012, at the address of 5301 W. Alexis Rd. Sylvania, OH: The employer failed to ensure an employee was adequately protected from a fall while working from a pump jack scaffold. The employee was exposed to a near 17 fall to the ground below during siding operations of a multi-family residential structure.
