WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD WHICH WAS COINTAINED IN OSHA INSPECTION NUMBER 314979311, CITATION NUMBER 01, ITEM NUMBER 004(B), ISSUED ON 07/27/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 134 WEST MAIN STREET, TARRYTOWN, NY 10591, THE FINAL ORDER DATE OF WHICH WAS 08/16/2011.
