(a) R M Davis, Inc. on or about April 18, 2012, at the address of 432 Wentz St. Tiffin, OH: The employer did not ensure an employee was protected from head injuries while working below an employee working above. The employee walking/working below an employee performing sign installation from the roof level was exposed to potential impact injuries caused from falling material and/or equipment.

The employee working in this condition was exposed to an approximant 7.5 ft fall to the ground below.
