29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Construction Site - On or about April 13, 2012 seven employees were exposed to multiple fall and puncture hazards while working. The employer did not clear the work area of all unnecessary tripping hazard. Employees were exposed to falling on piles of lumber with protruding nails.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) Roof - On or about April 13, 2012 four employees were exposed to a fall hazard while working on a roof without personal fall arrest system. One employee was working at the peak of the roof 31 feet 2 inches above the ground. The pitch of the roof was a 10/12 ratio.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. (a) Front of House - On or about April 13, 2012 one employee was exposed to a fall hazard while using an aluminum extension ladder that was not secured to the building nor was the ladder extended 3 feet above the landing. The ladder was used to gain access to a 13 foot high steep roof. (b) Right Side of House - On or April 13, 2012 two employees were exposed to a fall hazard while using an aluminum extension ladder that was not secured to the building nor was the ladder extended 3 feet above the landing. The ladder was used to gain access to a 12 foot high steep roof.
