29 CFR 1904.32(b): An Annual Summary of all recordable work-related injuries and illnesses (OSHA Form 300A), was not completed in detail as required by the regulation:    a) On or about March 27, 2012, at the above located workplace, the employer's OSHA 300A form for 2011 was not completed in detail as required by 1904.4 through 1904.32.   In that:    i) The number of hours work on the 2011, OSHA 300A, included approximately 12,000 hours worked by an outside contractor.     ii) The 2011, OSHA 300A, was not properly certified by a company executive.
