The material that was being applied was identified as Cafco 300, a powder identified as an eye irritant.  Employer did not provide the employee with effective eye protection.    Observed on or about 4/18/2012.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employer did not provide employees with effective information and training on hazardous chemicals in their work area, nor were MSDS sheets or any other information available on-site to employees for this substance.   Observed on or about 04/18/2012.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
