29 CFR 1926.252(a):  An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):    Jobsite located at 105 Summer Street, Torrington, Connecticut:  A chute was not used when debris was thrown from a third floor window opening to a roll-off container sitting on the ground approximately 23 feet below.

29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    Jobsite located at 105 Summer Street, Torrington, Connecticut:   Laborers were elevated forks or pallet on the forks of the CAT skid steer, serial number 3438A during demolition operations.  The Safety Manual for the skid steer prohibited lifing personnel with the equipment or riders on the equipment.      Abatement Note:  This requirement also applies to forktrucks.  If lifting personnel is allowed by the vehicle manufacturer, properly designed and secured lifting platforms must be used.

29 CFR 1926.850(a):  An engineering survey was not performed by a competent person to determine the conditions of the framing, floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:    Jobsite located at 105 Summer Street, Torrington, Connecticut:  The company had not conducted a demolition survey prior to starting interior demolition work.  A skid steer and forktrucks were brought up to the second and third floors to lift personnel, pull down electrical conduit and piping, to smash out the bricks around a window opening and to run under flooring to loosen large sections for removal.  Rotten areas in the flooring were observed and a large, structural roof beam was observed to be cracked in half during the inspection.

Second Floor:  Window wall opening approximately 14 feet above the ground.  B.  Third Floor: Window wall opening approximately 23 feet above the ground.
