29 CFR 1910.303(b)(2): Listed or labeled equipment was not used or installed in accordance with instructions included in the listing or labeling: a) During the period of March 19, 2012 and April 19, 2012, at the above located workplace employees in the lunch room were allowed to use a 120 volt Quasar microwave that was missing a grounding pin. b) During the period of March 19, 2012 and April 19, 2012, at the above located workplace employees in the lunch room were allowed to use a 120 volt Sharp commerical microwave that was missing a grounding pin. c) During the period of March 19, 2012 and April 19, 2012, at the above located workplace employees at welding station 15 were required to work with and around a 240 volt disconnect box which was not approved to be used in wet or damp locations. d) During the period of March 19, 2012 and April 19, 2012, at the above located workplace employees at welding station 28 were required to work with and around a 240 volt disconnect box which was not approved to be used in wet or damp locations.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosure. In wet locations the enclosure was not weatherproof. a) During the period of March 19, 2012 and April 19, 2012, at the above located workplace, employees at welding station 15, used a 240 volt disconnect box in a wet location the enclosure was not weatherproof. b) During the period of March 19, 2012 and April 19, 2012, at the above located workplace, employees at welding station 28, used a 240 volt disconnect box in a wet location the enclosure was not weatherproof.
