29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about April 19,2012 on the west side of the theater employees were exposed to fall hazards while working from a work platform elevated by a high lift all terrain fork truck.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 19, 2012 on the west side of the theater, employees were exposed to a fall hazard of 13 ft. while working from a scaffold platform.
