This violation occurred on or about April 20, 2012, in the shop area where employees operated, serviced, and set-up bending machines and a shear machine to fabricate metal re-bar products and a hazardous energy program was not established.

This violation was observed on or about April 20, 2012, where an employee was allowed to operate Caterpillar and Hyster forklifts throughout the facility and the employees were not trained and evaluated by the employer.
