29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): (a) Cable Inc at 440 S Front Street, Sterling, CO 80571: On and before April 18, 2012, the employer did not ensure that the required workplace hazard assessment for company operations. Employees conduct operations such as, but not limited to, pouring rope sockets and hydrotesting and slickline operations requiring access to well heads. These conditions potentially expose employees to hazards such as, but not limited to, inhalation hazards, burns, and falls. Abatement Note: The written certification that shall identify the following: 1. The workplace evaluated; 2. The person certifying that the evaluation has been performed; 3. The date(s) of the hazard assessment; and 4. The document as a certification of hazard assessment. 29 CFR 1910.253(b)(4)(i): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: (a) Cable Inc at 440 S Front Street, Sterling, CO 80751: On and before April 18, 2012, the employer permitted an oxygen cylinder to be stored adjacent to oil and grease in the East shop of the facility. The employer uses an oxygen cylinder for maintenance purposes. The manifold containing the oxygen cylinder is adjacent to the benchtop and chemical storage areas. This is less than 20 feet and there is not a noncombustible barrier separating the cylinder from the chemicals. Chemicals include, but are not limited to, Shell Rotella T3 Heavy Duty Engine Oil 15W-40, Mystik Multipurpose Grease, and Farm and Ranch SAE 80W-90 Gear Oil. This condition potentially exposes employees to a fire and explosion hazard. (b) Cable Inc at 440 S Front Street, Sterling, CO 80751: On and before April 18, 2012, the employer permitted two oxygen and one acetylene tank to be stored adjacent to each other in the slickline trailer storage area of the West shop. The tanks were capped and were not securely fixed to a cart or vertical surface. This condition potentially exposes employees to a fire and explosion hazard.
