29 CFR 1926.501(b)(5):     "Formwork and reinforcing steel." Each employee on the face of formwork or reinforcing steel shall be protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems.    The employer failed to ensure that every employee was protected against fall hazards associated with formwork and reinforcing steel where the employees are working more than 6 above the ground below.  The most recent example of this was found on the Highway 34 Bridge Project job site located near Harlan Lewis and LaPlatte Dr. in Bellevue, NE 68123 where the employer had an unprotected employee tying in 60 long rebar while they were building a form for in place concrete casting and subsequently the employees lack of fall protection lead to him suffering serious injuries as a result of falling 16 to the ground below.

29 CFR 1926.502(d)(21):     Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service.    The employer failed to ensure that the employees personal fall arrest harnesses were being inspected prior to use.  The most recent example of this was found on the Highway 34 Bridge Project job site located near Harlan Lewis and LaPlatte Dr. in Bellevue, NE 68123 where the employer had employees using unserviceable fall arrest harnesses on the job site as a means of fall protection.

29 CFR 1926.503(a)(1):     The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    The employer failed to provide employees with adequate fall protection training necessary to recognize potential hazards associated with conducting form work where the employees are working more than 6 above the ground below.  The most recent example of this was found on the Highway 34 Bridge Project job site located near Harlan Lewis and LaPlatte Dr. in Bellevue, NE 68123 where the employer has untrained employees tying in 60 long rebar as they were building a form for in place concrete casting and subsequently an employees lack of hazard recognition skills contributed to his failure to maintain 100% fall protection; as observed in his suffering serious injuries as a result of falling 16 to the ground below.

29 CFR 1904.29(b)(3):     How quickly must each injury or illness be recorded? You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.    The employer failed to record an employees injury as a result of a work place accident as required in this subpart.  The most recent example of this was found on the Highway 34 Bridge Project job site located near Harlan Lewis and LaPlatte Dr. in Bellevue, NE 68123 where the employer had an employee fall 16 to the ground below suffering substantial injuries whereby the employee was taken by life flight to the hospital and treated for the injuries resulting in him being held for more than three days.
