29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. Job Site - As the controlling employer, they did not insure that midrails were left in place on the second floor balcony, exposing employees to a ten foot fall.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: Inn by the Sea Job Site - An employee used a table saw without either a ground fault circuit interrupter or an assured equipment grounding program. Hebert Construction, LLC, was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection number 315061077, Citation number 1, Item number 1, and was affirmed as a final order on 3/25/11, with respect to a workplace located at 95 Campus Avenue, Lewiston, ME.
