29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length, extended over its support more than 12 inches. On or about April 16, 2012, on the south side of the building, employees engaged in masonry activities on a residential building from a scaffold with the end of the platform decking extended over 12 inches past the bearer exposing the employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about April 16, 2012, on the south side of the building, the employer, whose employees were engaged in masonry activities on a residential building from a scaffold, did not provide a scaffold ladder where employees could access the upper landings. The employees were exposed to a fall hazard of up to 13 feet to the ground below.
