29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above thefloor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch. On or about 12 April 2012 and times prior thereto, two (2) fan blades Fan # H-0014 (Main Building Heater) and Fan # D2P-04 (Electrical Room) unguarded in the sewage treatment plant. This exposed employees to struck by hazard.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles may have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 2.44 m (8.0 ft) above the floor may have exposed terminals. On or about 12 April 2012 and times prior thereto, in the Waste Operators office, electrical wires in wall recepticle bnot covered. This exposed employees to electrical shock. No hospitilization anticipated.
