29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    Jobsite:  The employee, who was exposed to the hazards of cuts and puncture wounds on the feet from the scrap steel studs and construction debris scarattered on the floor was not wearing appropriate protective footwear.

29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    Jobsite:  The employee, who was working/walking  in and around areas inside the building where active overhead work , such as (but not limited to) plumbing, ceiling, and electrical work was in progress was not protected from head injuries by the use of appropriate and proper protective helmet.

29 CFR 1926.100(b): Helmets for the protection of employees against impact and penetration of falling and flying objects did not meet the specifications contained in American National Standards Institute Z89.1-1969, Safety Requirements for Industrial Head    Jobsite:  The "hard hat", worn by the employee who was working/walking in and around the areas inside the building where active overhead work, such as (but not limited to) plumbing, ceiling, and electrical work was in progress was not only worn in backward, but also did not have the inside protective webbs.
