29 CFR 1926.1053(b)(1): On portable ladders that were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. 39 Granby Rd, Worcester MA On April 20, 2012, two employees installing roofing materials on a roof with eave 23ft 1-5/8in above sidewalk were exposed to fall hazards while mounting and dismounting three ladders that neither extended 3 feet above landing surface nor had grasping devices installed.
