29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about April 12, 2012, at drilling Rig # 1, the railing was missing at two different locations where employees work. This condition exposed the employees to a fall hazard of approximately 18 feet from rig floor to ground.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: On or about April 12, 2012, at Rig # 1, the mud pit stairs were missing the railing on both sides. This condition exposed the employees to a fall hazard of approximately 5 feet 7 inches to the ground below.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On or about April 12, 2012, the pipe racks north of the catwalk on Rig # 1, were not chocked or pinned; the racks were missing the pins on both sides of the rack. This condition exposed the employees to potential struck-by/caught-in-between hazards.
