29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle. On or about April 18, 2012 and at times prior thereto, employees operating the EZ Scrapper #1717, 336 Assembly Fixture and Pearl Carpet Welder machines were exposed to having their hands and fingers in the danger zone during the operating cycle. This was occurring because the individual operator's hand controls were not equipped with a control arrangement requiring concurrent operation of the individual operator's hands.
