29 CFR 1926.451(d)(9): The load end of wire suspension ropes shall be equipped with proper size thimbles and secured by eyesplicing or equivalent means. a. At the job site, the employer failed to ensure that the load end of the choke cable on the left side connecting the Swing Stage to the structural I-beam had thimbles, there-by exposing employees to a fall hazard. b. At the job site, the employer failed to ensure that the load end of the choke cable on the right side connecting the Swing Stage to the structural I-beam had thimbles, there-by exposing employees to a fall hazard.

29 CFR 1926.451(d)(10)(iv): Ropes shall be inspected for defects by a competent person prior to each workshift and after every occurrence which could affect a rope's integrity. Ropes shall be replaced if any of the following conditions exist: Abrasion, corrosion, scrubbing, flattening or peening causing loss of more than one-third of the original diameter of the outside wires. a. At the job site, the employer failed to ensure that the choke cables were protected and/or replaced on the left side, due to the flattening caused by the sharp angles of the beam they were connected around thereby exposing employees to a fall hazard. b. At the job site, the employer failed to ensure that the choke cables were protected and/or replaced on the right side, due to the flattening caused by the sharp angles of the beam they were connected around thereby exposing employees to a fall hazard.
