29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was used prior to repair; a) Spider box on the second floor Northeast corner: On 4/17/2012, and at times prior thereto, employees were exposed to possible electric shock hazards when using power from a 208 volt 50 amp spider box that had a broken female cord cap receptacle. Note: Abatement certification is required.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: a) Spider box on the second floor Northeast corner: On 4/17/2012, and at times prior thereto, employees using cord and plug powered tools energized from the spider box were exposed to possible electrical shock hazard because the power supply cord (baloney cord) was pulled loose disrupting power to the box. Note: Abatement certification is required.
