29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 4/16/12 and 4/17/12 on the 6th floor near column E-3 at bridge, on construction site at Ellicott & Goodrich Streets (UB Space Build-out), Buffalo, NY: The plug on the cord of a large portable fan used to dry a wall after taping lacked a ground prong. The employer owned the fan, brought it to the jobsite approx. 2 months ago, did not perform perform frequent & regular inspections of the fan to ensure that it was in safe operating condition, and allowed employees of other contractors to use it. NO ABATEMENT CERTIFICATION REQUIRED
