29 CFR 1926.1053(b)(15):     Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use: a) Front Porch Roof - A laborer was using a 20 ft, portable metal extension ladder to bring material to a carpenter and the skid resistant foot was missing, on or about 04/12/12.

29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) Exterior Front, Ground Level - A carpenter was standking below carpenters that were installing Dur-rock to exterior facade of the structure, and not wearing head protection, on or about 04/12/21. District Properties was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 313887796, citation number 1, item number 1, and was affirmed as a final order on 01/11/11, with respect to a workplace located at 4922 D Street, SE Washington DC.

29 CFR 1926.501(b)(13):     "Residential construction." Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provisionin paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it isinfeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets therequirements of paragraph (k) of 1926.502:  a) Exterior Front Porch - A carpenter and laborer were observed standing on the roof while installing Dur-rock to the exterior facade of the structure, and exposed to a 13 ft, fall hazard, on or about 04/12/12.     District Properties was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315679357, citation number 1, item number 1, and was affirmed as a final order on 08/05/11, with respect to a workplace located at 3355 Frankford Street, SE Washington DC.    District Properties was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314924721, citation number 1, item number 1, and was affirmed as a final order on 11/18/2010, with respect to a workplace located at 5508 4th Street, NE Washington DC.
